  extrap_c0 doc: document the manual switchover q (merge_q)

Follows 6dd62b878. Adds the "switchover q" field to the reference-splice description
(pin the merge to a chosen q, blank = automatic) and the scripted merge_q directive to
the directive list. Directive audit back to 20/20; bare-<p> imbalance unchanged.
