  extrap_c0 manual: rewrite modes section for the two output-convention checkboxes

Replace the single "Absolute-scale mode" description with the two independent
checkboxes (output on reference scale; splice reference at high q), note they
combine freely with any concentration model into four behaviors, and update the
concentration-model, fit-broadening, regression-plot and output-naming sections
to match (including the new self-describing name tokens).
